I’ve got a lot to learn, my friends. I’m still learning much about my medical speciality for which I have been “board certified”. That means I know things, right? I’ve been working on this diligently for YEARS. Well, each day I discover how much more I need to learn. I have a keen interest in economics as well, but that road leads me to frustration and despair. I hung up those books in 2015. May need to get back to them. I am not going to be knowledgeable at politics, healthcare reform, foreign policy, in such a way that I can debate it honestly with thorough facts rather than my emotions and biases. I can’t site evidence like I can with medicine. I can’t be an expert at all things. Many people ask me for my thoughts on things that are outside of my scope. I answer, I don’t know. Why? Because I don’t know, and I’ve accepted that. That has made people mad in the past and that’s okay. I teach medicine for free. That’s what I do. That also doesn’t mean I am not working to fill the gaps and improve. Too many know-it-alls in our society today imposing their thoughts and beliefs on others. No one learns or responds from being spoken down to or humiliated. Polarizing others does not improve society outcomes. It does not lead to conversations. It leads to yelling and anger. We are all in this life together whether we like it or not. We need to treat each other better. ❤️

NORMAL FETAL CARDIAC CIRCULATION Fetal cardiac circulation originates via the placenta: •Oxygentated blood (red arrows ) flows from the Mother across the placenta, through the umbilical vein (UV ) to the right atrium (RA ) – and is directed toward the interatrial septum and across the foramen ovale (PFO ) (white curved line ) to the left atrium (LA ), across the mitral valve to the left ventricle (LV ), through the aortic valve to the aorta (red outline ), to the systemic circulation. •Deoxygenated blood (blue arrows ) flows from the fetal systemic circuclation through the superior vena cava (SVC ) to the RA – and is directed across the tricuspid valve to the right ventricle (RV ), through the pulmonary valve to the pulmonary arteries (blue outline ), to the lungs. Blood returns to the heart via pulmonary veins (PV ) and mixes with blood in the LA. •In utero the ductus arteriosus (purple outline ) shunts deoxygenated blood (blue arrows ) to the aorta (Ao ) which mixes with oxygenated blood in the aortic arch. The mixture of oxygenated and deoxygenated blood (purple arrows ) is delivered to the systemic circulation, supplying abdominal visceral organs via the descending aorta (DAo ) and returns to the placenta via the umbilical artery (UA ), ultimately to Maternal circulation.

Very excited to announce that I’ve accepted a position as Assistant Professor of Medicine at @bonduniversity on the Gold Coast commencing next month. I have been working towards this for a long time & am absolutely thrilled to have this exciting opportunity to work with a great team & carry on my areas of research. Moving in the midst of border closures and pandemics is certainly proving challenging...what a juggle 🤹♀️ So...where’s the best coffee on the Gold Coast & what does one do with two children whilst in hotel quarantine?!! (Srsly. How are we gonna get through that?!! )
I remember in undergrad, my 'terrifying' Physics prof would around the class and pick students on the spot to answer questions. . I would always try to make my self smaller, shrinking my seat to avoid being "called upon", and sometimes would blank when picked because of nervousness, being afraid of how I'd come across as less prepared to my peers. . ➡️ I now appreciate that he did this to stimulate interesting discussion and learning - not because he wanted to humiliate or embarrass his students. As a PA student and now practicing PA, being put on the spot to answer questions during class, on rotations, and even on the job is common place. This is known as a Socratic-style of teaching which is a "form of cooperative argumentative dialogue between individuals, based on asking and answering questions to stimulate critical thinking and to draw out ideas and underlying presuppositions" (wikipedia ). In layman's terms, this is called "pimping". 𝐇𝐚𝐯𝐞 𝐲𝐨𝐮 𝐞𝐱𝐩𝐞𝐫𝐢𝐞𝐧𝐜𝐞𝐝 𝐭𝐡𝐢𝐬 𝐛𝐞𝐟𝐨𝐫𝐞? (𝐖𝐡𝐞𝐭𝐡𝐞𝐫 𝐛𝐞𝐟𝐨𝐫𝐞, 𝐝𝐮𝐫𝐢𝐧𝐠 𝐨𝐫 𝐚𝐟𝐭𝐞𝐫 𝐏𝐀 𝐬𝐜𝐡𝐨𝐨𝐥? ) 𝐰𝐡𝐚𝐭 𝐝𝐢𝐝 𝐲𝐨𝐮 𝐝𝐨? In fact, when I underwent this last recently I was asked several questions with an attending in front of a patient: "What are 5 causes of AVN in the hip? Describe findings in Trendelenburg gait? What do you see on XRay? What treatment do you propose? Explain your rationale." . I explained to the patient although it may come across as aggressive, the intent is not to humiliate or embarrass, but to use a starting point for learning. Although when you first experience "pimping" and don't immediately know the answer - be honest that you don't know (this can happen even despite all of your preparation! ). You may feel "less than" and embarrassed for not knowing, but the silver lining is that you now know what your gaps are. You can now go home, and review, so that the next time they ask the question (and the will! ) you know the answer. Be humble, medicine is an ocean and you won't catch it all the first or even second time you're going through material. It will come up with practice, experience, repetition and time. 📚❤️
